8. Copy of Marriage Certificate or Birth Certificate (single parents only) (valid for 6 months)

9. Medical Report – Father (valid for 6 months)

10. Medical Report – Mother (valid for 6 months)

11. Financial Statement (Affidavit)

12. Home Study (including attached Family Photos, Home Study Agency License, Home Study Social Worker License or Diploma)

13. Parents Educational and Medical Commitment

14. Adoption Agency Post-Adoption Report Commitment

15. USCIS Approval (I-171H form)

16. Employment Letter – Father

17. Employment Letter – Mother

18. FBI Clearance Letter – Father (valid for 6 months)
